Name of Product: Counterfeit "Square D" Circuit Breakers Units: About 43,600 Distributor: Miami Breaker Inc. (MBI), of Miami, Fla. Importer: General Breakers and Panels Inc., of Miami, Fla. Hazard: The recalled circuit breakers labeled "Square D" or "SQD" have been determined to be counterfeit by Square D and can fail to trip when they are overloaded, posing a fire hazard to consumers. Incidents/Injuries: None reported. Description: The counterfeit circuit breakers are marked as Square D in the QO series. The models are QO115 (about 7,800 units), QO120 (about 26,300 units), QO130 (about 3,200 units), QO215 (about 350 units), QO220 (about 720 units), QO230 (about 1,780 units), QO240 (about 1,130 units), QO250 (about 1,150 units) and QO260 (about 1,150 units). Genuine Square D QO breakers have the following: (1) the amp rating written on the handle in white paint on the front of the breaker, (2) the Square D insignia molded onto the breaker side; (3) a yellow chromate clip with half of the top of the clip visible. If your breaker does not match this description, it could be counterfeit. Sold to: Electrical product distributors and wholesalers nationwide from March 2005 through July 2006. Single pole breakers sold for between $3 and $4, double pole breakers were sold for between $8 and $9. Hazard: The fuel line on the rear-mounted fuel tank is too long and can come in contact with moving parts while the tractor is in use, posing a fire hazard. Incidents/Injuries: The firm has received 53 reports of fuel line damage. No injuries have been reported. Description: This recall involves gas-powered Husqvarna riding lawn tractors designed for residential use. The lawn tractor's model and serial numbers are printed on an identification plate located underneath the tractor seat. Models and serials numbers included in the recall are: GTH27V52LS lawn tractors with model numbers 96043009100 and 96043009900, with serial numbers 010810A00XXXX through 05061000XXXX. Sold by: Husqvarna authorized dealers nationwide from January 2010 to May 2010 for about $3,000. Manufactured in: U.S.A. The recall is being initiated as a precautionary measure because these products may have been manufactured under conditions that did not sufficiently comply with current Good Manufacturing Practices (cGMPs). These practices help assure that drug products meet required quality standards for quality, purity and potency. The potential for serious medical events is remote and other than one initial complaint concerning a contaminated cylinder which triggered the initial recall, Airgas South is not aware of any other confirmed events involving cylinder contamination. The Company has no reason to believe that other contaminated cylinders will be found. cGMP refers to the Current Good Manufacturing Practice regulations enforced by the US Food and Drug Administration (FDA). cGMPs provide for systems that assure proper design, monitoring, and control of manufacturing processes and facilities. The recall involves high-pressure medical cylinders containing oxygen, nitrogen, air, nitrous oxide, carbon dioxide, and drug mixtures with expiration dates of July 13, 2010 through July 13, 2015; portable Walk-O2-Bout®; oxygen cylinders with expiration dates of July 13, 2010 through July 13, 2012, and liquid cylinders (VGL) containing oxygen, nitrogen, carbon dioxide, and nitrous oxide with expiration dates of July 13, 2010 through July 13, 2011. The recall affects customers located in Tennessee, Alabama, and Georgia. After an initial completed recall, further investigation at the Chattanooga facility identified cGMP issues and led to the decision to voluntarily recall additional products as a precaution. As an additional precaution, the Chattanooga facility has ceased medical filling operations until the investigation is complete and the root causes of the issues identified have been resolved. Airgas South’s expanded recall notice asks customers who locate any cylinder(s) within the groups and with the expiration dates affected by the recall to clearly tag and quarantine those cylinders, and contact their local Airgas South representative. Hazard: The pin connector can fail and prevent a door from being unlocked from the inside, posing an entrapment hazard to consumers. This failure can lead to the inability to vacate a location in an emergency. Incidents/Injuries: Stanley Convergent Security Solutions has received three reports of the pin connectors failing. No entrapments have been reported. Description: The recalled pin connectors were distributed in installation kits (part number 8Z-40010751) for use with Sonitrol-brand ACCESS4-S and ACCESS4-SULC four-door access control modules. The recalled connectors include two-pin, eight-pin and 17-pin models, all of which have “EB560” imprinted on the end.
